Nichole is Your Mental Health Expert. She practices Integrative Psychiatry to help you identify and treat the underlying cause(s) and contributors to symptoms of depression, anxiety, brain fog, fatigue, ADHD, insomnia, mood disorders, and other contemporary mental health conditions.  

Integrative Psychiatry is a personalized approach to mental health that uses functional medicine and traditional psychiatry to identify and treat core imbalances which affect your brain, body, and life.

 

Nichole will search for underlying triggers that are contributing to your symptoms and tailor the intervention to your specific needs as an individual. She realizes that there is no separating the brain and the body and may use counselling techniques, supplements, lifestyle changes and/or medication to treat your condition.

Nichole seeks to promote holistic mental health and create hope, health and healing by working with you.

The Answers You Need

WHAT CAN I EXPECT IN THE FIRST VISIT?

In your Initial visit Nichole will evaluate your full medical, psychiatric, psycho-social, and family history in conjunction with your present symptoms. An accurate medical history is important to obtain at this first visit because certain medical conditions can present with psychiatric symptoms. Prior to this visit you will be sent a link asking you to register with our patient portal, please remember to complete the New Patient Questionnaires in your online account. During this session, your treatment goals will be discussed in depth and a full treatment plan (including medications, complementary and alternative medicine, and/or psychotherapy if needed) will be prescribed.

DO YOU ACCEPT INSURANCE?

At this time we are in-network with Regence Blue Cross Blue Shield of Oregon, Moda and PacificSource.  If you have a different insurance plan and would still like to be seen by Nichole Slykhous you can either use your out-of-network benefits or pay the cash rate at the time of the appointment. Some patients have very good out-of-network benefits. We can also provide a Superbill receipt with a summary statement for psychiatric services for you to request reimbursement from your health savings account (HSA, FSA or HRA).

 

WHAT QUESTIONS SHOULD I ASK MY INSURANCE?

You can use this form to speak with your insurance company prior to your appointment to verify your benefits so that their are no surprises when you receive your bill.

CAN I PAY USING MY HSA, FSA, or HRA?

Yes! Psychiatric care is eligible for reimbursement with a flexible spending account (FSA), health savings account (HSA), or a health reimbursement arrangement (HRA). You can pay for your appointment with your FSA, HSA, or HRA debit card at the time of the appointment and/or we can provide you with a Superbill receipt to request reimbursement from your savings account.

HOW MUCH DOES AN APPOINTMENT COST?

If you would like to use your insurance for your appointments your cost will vary based on your plan and coverage. Please use our insurance verification form to determine your expected co-pay and co-insurance.

 

If you would prefer to pay cash the fee rates are as follows:

*There is a same day cash payment discount of 20%

Initial In Person Psychiatric Evaluation 60 minutes: $428

Follow-Up Appointment 30 Minutes: (virtual or in person): $196

Virtual Psychiatric Consultation 75 minutes: $495

  • If you live outside of Oregon and would like a consult regarding your current diagnosis and medication plan you can book a consultation with Nichole. You should have an existing provider in your home state who will provide your ongoing care. 

Additional Services:​​

  • Additional Forms requested outside of appointments: $50 per form.

​*Simple copies of diagnosis letters, and your notes are free.

WILL I NEED ONGOING CARE?

In your initial intake appointment we will establish your goals and create a treatment plan. While some patients benefit from ongoing psychiatric care other patients may prefer to be seen less often. When you are ready, you can transfer your medications or treatment plan to your primary care provider or therapist, with the option of making a follow-up psychiatric appointment as needed in the future.
